RN - Hospice Job Opening

Responsible for planning, coordinating and documenting care for an assigned caseload of patients. Utilizing physicians' orders and professional skills the Hospice RN in conjunction with the Case Manager develops and implements a home health plan that meets each patient's specific needs and is in compliance with Joint Commission, federal and state regulations, reimbursement guidelines as well as KershawHealth policies and procedures. Participates in performance improvement and continuous quality improvement activities.

SC RN licensure, Good Driving Record, BLS
2+ Years of Acute Care Experience or 1 yr Home Health or Hospice.
