Summary Supports the Mission, Philosophies and Goals of the Agency, coordinates service delivery for caseload by performing the following duties.

Essential Duties and Responsibilities:


  • Facilitates Person Centered Support Plans and PCSP updates.
  • Participates in BASIS meetings.
  • Participates in WORKS assessments.
  • Maintains HCBS documentation for individuals receiving services.
  • Maintains WORKS documentation for individuals receiving services.
  • Writes, implements and routes Person Centered Support Plans.
  • Reviews written Person Centered Support Plans.
  • Participates in admission/discharge, transfer meetings and completes all documentation from those meetings.
  • Coordinates disability benefits and funding for recipients.
  • Coordinates mental health consultations.
  • Functions as liaison between service providers and recipients.
  • Completes and maintains Targeted Case Management Documentation.
  • Serves as advocate by maintaining contact with families, guardians and interested parties.
  • Conducts regular reviews or visits at home and/or work.
  • Connects individuals to crisis interventions
  • Writes, routes and monitors Risk Assessments, Behavior Support Plans, Needs Assessments and Individual Justice Plans.
  • Monitors programs and supports to ensure they are being implemented.
  • Coordinates and attends support team meetings with schools or alternate agency/programs.
  • Completes annual Quality Assurance Surveys at the recipient's residence.
  • Initiates and maintains communication with SRS, Fiscal Management providers and CDDO representatives.
  • Must complete trainings as required within specified timeframe. (See Employee Handbook Section 2.11) Complies with all agency policies and procedures including HIPAA/Confidentiality policies.

Qualifications

Education/Experience/Licenses: Bachelor's degree (B. A.) from four-year college or university is preferred; six months experience is required. Knowledge of Database software; Excel Spreadsheet software and Microsoft Word Processing software. Valid Kansas driver’s license; Targeted Case Management certification or must have the ability to receive certification within 60 days. Must be able to travel 25% of the time.
