VAST Case Managers Job Opening

We are seeking two experienced case managers with experience working with the Veteran population. Must have the following credentials and experience...

• BA/BS in Psychology, Human Services, or related field with 5 years minimum experience
• Experience working with multiple populations, including Veterans, youth, LGBTQ clients,
  African Americans, Latinos, persons with HIV/AIDS, homeless, substance
  abuse, and survivors of sexual violence, including domestic violence.
• Demonstrated cultural competency working with African-Americans and/or Hispanic
• Experience with HIPPA regulations a must
• Experience as a platform trainer, providing instructions in human services
• Computer literacy in Microsoft Office (Word, Excel, Outlook).
• Demonstrated project management experience
• Resourceful and willing to travel throughout north region of New Jersey
• Reliable transportation a must

Key Performance Elements/Essential Functions:

• Assess client needs through interviews and chart/history review
• Maintain active caseload roster, charting progress notes and attendance
• Prepare reports and maintain confidential individual case summaries
• Refer participant to other programs as needed
• Establish and maintain relationships with social service agencies
• Manage special projects as assigned, as well as create action plans as required
• Maintain all confidential client records and databases to ensure HIPAA compliance
