Overview

The Home Health Aide is responsible for providing, restoring, maintaining, and promoting the health status of the clients through home support, medical monitoring, client transportation services, and assistance with activities of daily living.

Responsibilities

  • Assists in meeting the goal and objective of the client’s treatment plan.
  • Must be knowledgeable of the policies and procedures of this company and follow them in the daily care of clients.
  • Strictly adheres to the principles of Infection Control.
  • Keeps Nursing Supervisor and/or Administrator informed of changes in client’s condition.
  • Provides support and assistance for activities of daily living according to procedure to include:
    • Bathing/showering
    • Personal grooming
    • Personal hygiene
    • Meal planning and preparation
    • Assisting clients in and out of bed
    • Assisting with ambulation
  • Provides home support and assists in maintaining a healthy environment by performing the following duties:
    • Light cleaning
    • Light laundry
    • Shopping
    • Home safety
  • Assists in monitoring the medical status of client to include:
    • Monitoring of vital signs
    • Monitoring medications
    • Basic recognition of medical problems being experienced by the client
    • First aide for emergency situations
  • Assists in arranging for, or providing transportation for client for appointments for the following reasons:
    • Doctor’s appointments
    • Therapy appointments
    • Any appointments that assist to better promote well-being for the client
  • Daily record keeping which includes, but is not limited to:
    • Documenting vital signs, recognizing any abnormalities
    • Documenting basic ADL’s and improvements vs. problems
  • Provides for care of the client’s hair, nails, and oral hygiene according to procedure as evidenced by observation and client comments.
  • Attends staff meetings and participates in the planning, evaluation and coordination of total program efforts.
  • Contributes to a positive work environment and the overall team effort of the location.
  • Additional duties and responsibilities as required or directed.

Qualifications

  • High school diploma or general education degree (GED) preferred; and three to six months related experience and/or training preferred; equivalent combination of education and experience acceptable. Must be able to pass competency testing.
  • Ability to read, write, and speak English.
  • Ability to read and comprehend simple instructions, short correspondence, and memos.
  • Ability to write simple correspondence.
  • Ability to effectively present information in one-on-one and small group situations to clients and other employees of the organization.
  • Interpersonal skills to perform care to clients ranging in age from adolescence to death, on a continuous basis.
  • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
  • Ability to compute rate, ratio, and percent and to draw and interpret bar graphs. (Necessary for intake, output and calorie counts.)
  • Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form.
  • Ability to deal with problems involving several concert variables in standardized situations.
  • Current driver’s license may be required.
