Care Manager Job Opening

Looking for Registered Nurse, Licensed Social Worker, Certified Case Manager, Nurse Practitioner, Licensed Professional Counselor, BA or MA in Gerontology or Psychology, Sociology, Mental Health Counseling and Human Services, or LPN (3 years) to meet with senior citizens in their homes to help assist with Care Coordination & Care Management. No hands-on care. This is to conduct assessments only. Meet with senior citizens once a week in their homes for about an hour per visit. Pay Rate: $80-$100 per senior citizen per month. Information must be uploaded to our system same day as your visit. Please let us know if you are interested.
