Job Summary:
Under supervision, plans, manages, and evaluates social services programs. Administers and coordinates the services and program activities in dependency, mental illness, youth development, or physical or developmental disability programs.

Job Description:

Coordinators will serve as Pre-release case manager by participating in reentry case planning and collaborating with agency partners and community service providers up to one year before release. Coordinators will participate in the planning and implementation of individual reentry plans and engage all stakeholders including the individual in prison, prison staff, releasing authorities, supervision authorities, victims, the individual’s families and significant others, human service agencies, and volunteer and faith-based organizations in the process.

In-Reach Management and Implementation Planning

1. Conduct interviews to compile current information regarding the reentry plan

2. Attend and provide appropriate input in scheduled treatment meetings
3. Provide and connect caseload to community resources availability prior to release.
4. Plan mentor meetings and training sessions that include educational materials pertinent to their needs.
5. Serve as contact for community service providers and supervision officers
6. Provide reports and case information to the DCS staff, as needed
7. Arrange services with the community supervision officer based on the identifiable need areas as indicated in reentry plan
8. Schedule individual inreach meetings with the caseload and help identify community resources and service providers to address the additional need
9. Build and maintain Case Planning and Service Delivery System with the resources needed to be successful in order to appropriately manage risks and build on strengths.
10. Consult with DCS Reentry Site Community Coordinator and the Transition Team to ensure smooth handoff from prison to community supervision.
11. Engage and educate key stakeholders about the plan to promote buy-in with community partners.

One of the most critical aspects of this position is oversight of case planning from the point of entry to prison to release on supervision. This process will be completed in coordination with prison-based counselors and programmers through returning and working with third party service providers to assure adherence to evidence based principles and practices.

Oversight of Re-Entry Plan


1. Serve as official guide for individuals transitioning from prison, corrections and field staff and community service providers by facilitating linkages and communication.
2. Define programs or interventions to modify individual dynamic risk factors as identified by a systematic assessment process.
3. Address the risks and needs and build on the identified strengths of each individual.
4. Provide a long-term road map to achieve continuity in the delivery of treatments and services, and in the sharing of information, both over time and across and between agencies

Follow Up and Modification

1. Encourage weekly engagement and support from returning steering teams in the county of residence.

2. Encourage employers to visit the correctional facility to meet with prospective employees before release

3. Engage community members and community-based services to act as intermediaries between employers and individuals seeking employment

Receives additional training, as required, to gain full proficiency and experience in all areas. Establishes and maintains relationships with other agencies and organizations in the community in order to meet community needs and prevent duplication of services. Evaluates the provision and quality of recommended services to assure compliance with program requirements and policies. Guides and oversees the distribution of resources. Identifies and/or manages grants, contracts, and other facets associated with the programs. Implements, coordinates, monitor and evaluate social services programs. Organizes community stakeholders and resources to identify proactive and effective solutions. Participates in the determination of organizational policies regarding such issues as participant eligibility, program requirements and program benefits. Provides policy and/or management guidance to representatives of the programs.

Qualifications:
Bachelor's degree in a related field from an accredited college or university. Note: Some positions require eligibility for licensure.
Preferred Qualifications - External:
Applicant should meet at least three or more of the following: Two years or more experience planning, organizing, and goal setting. Two or more years working with internal and external customers. Experience communicating with and gaining support from a wide variety of individuals from various socio-economic, cultural, ethnic, educational, and other diversified backgrounds. Two years or more working in a human service delivery area. Open to internal (DCS Employees) and external candidates.
