Responsible to the HSA/Department Head for discharge planning assignments. Coordinates personnel inside the facility and in the community. Interrelates and works effectively with all levels of authority.
Essential Job Duties:
The Discharge Planner will work under the direction of the Mental Health Director and Health Services Coordinator. The duties of the discharge planner are as follows:
? Serve as a liaison between County Alcohol, Drug and Mental Health, County Public Defender’s Office and the Jail Mental and Medical Unit.
? Meet with patients who are referred by Mental Health Specialists and gather information necessary to develop an aftercare plan
? Formulate/develop aftercare plan
? Document all interventions and patient’s response in patient’s medical record
? Provide patients with information needed for their individualized aftercare
? Participate in Quality Management meetings and Quality Management Peer Review activities and staff training
? Participate in case management meetings with on-site mental health providers
? Other duties as assigned by Health Services Administrator
? Post Orders, if applicable, per site contract.
Knowledge, Skills and Abilities Required:
? Licensed as a Marriage Family Therapist, Licensed Clinical Social Worker, or Psychiatric Technician with previous experience performing the duties of discharge planner
? Good communication skills
? Must obtain and maintain a security clearance
