Hospice Maui is looking for a licensed MSW to join our interdisciplinary care team as a Hospice Social Worker. As a non-profit organization serving Maui for over 35 years, Hospice Maui is honored to have earned the community’s trust as the respected hospice leader on Maui.
Job Summary:
Provide direct services to patients and caregivers in their homes, a 5-bed hospice hale (residential facility) and various community facilities. Assess psychosocial needs, provide patient advocacy, individual, family and group counseling, conduct crisis intervention, discharge planning and community referrals. Join a supportive team of care professionals.
Job Type: Regular full-time.
Minimum Qualification:
- Education: Master of Social Work (MSW) degree from a school of social work accredited by the Council on Social Work Education
- License: Hawaii State License (LSW); LCSW preferred
- Experience: One (1) year social work experience in a healthcare setting
- Demonstrate strong assessment and counseling skills along with cultural sensitivity and a deep commitment to patient advocacy at the end of life
- Personal automobile, current driver’s license and insurance
- Ability to work flexible hours to include rotation of on-call hours on weekends and holidays
Preferred Qualifications: Hospice or home health experience
