• Cheshire, ConnecticutAn Internal Medicine Group has opportunities for Licensed APRN's and PA's to work with geriatric patients in an office based environment in Greater New Haven.
GENERAL SUMMARY OF DUTIES:
The Advanced Practice Registered Nurse will manage primary and secondary care conditions, and evaluate tertiary care conditions. Primary care conditions are defined as common acute, chronic stable, or health care maintenance conditions; secondary care conditions are unfamiliar, uncommon, unstable, or complex conditions; and tertiary care conditions are acute, life-threatening conditions such as respiratory and cardiac arrest.
PRIMARY FUNCTIONS:
- Manage acute and chronic illness in collaboration with the primary care physicians
- Prevent re-hospitalization of residents in the skilled nursing facility
- Shorten length of stay in sub- acute rehabilitation patients
- Promoting quality of life by maintaining patients at the highest level of function.
- Bedside education to patients, families and staff as needed.
- Conduct weekly wound rounds with a holistic approach when necessary
- Assist patients and families with decision making at end of life
- Education to patients, families and staff on end stage disease process and expected outcomes at end of life
- Collaborate with the interdisciplinary team
- Promote cost effective delivery of care
REQUIRED KNOWLEDGE, CERTIFICATION / LICENSURE:
- The Advanced Practice Registered Nurse is masters or doctorate prepared
- Certified Nationally as a Nurse Practitioner through AANP or ANCC .
- The Nurse Practitioner must have possession of a valid ( Connecticut ) license as a Registered Nurse and an Advanced Practice Registered Nurse, and be certified as a Nurse Practitioner by an approved certifying body.
- DEA, and Ct controlled substance license
- Wound certification preferred
