Social Worker (LSW) or Registered Nurse (RN) Case Manager Job Opening

License Required: Licensed Social Worker (LSW) or Registered Nurse (RN)

Duties include, but not limited to:

• Interactive contact with, primarily, older adult clients by phone and in-person visits
• Coordinating and monitoring delivery of long term care services
• Traveling more than 50% of the time within 9 counties (Ashland, Crawford, Huron, Knox, Marion, Morrow, Richland, Seneca and Wyandot) with mileage reimbursement
• NO evenings, NO weekends, NO 'on-call' time
• Knowledgeable in Word and Excel
