This candidate will be part of a team of professionals working to maintain the highest physical, emotional, and psychosocial quality of life for residents in a skilled nursing facility. He/she will complete social histories, psychosocial assessments, develop plans of care, assist with discharges, network with other agencies to provide support for our residents.
