NP - Full Time - Long Term Care Job Opening

We are searching for a Nurse Practitioner in the long-term care/skilled nursing setting in Lewes/Seaford, DE 19951.

The hired Nurse Practitioner will be responsible for managing care for Medicare Advantage members in the Long-Term Care Facility setting.

Must have qualifications/experience:

• NP License


• DEA


• Board Certified


• At least 1 year of NP work experience - No New Grads accepted at this time

Preferred Experience:

• Long Term Care experience


• AGNP certification

Responsibilities:

• conduct Complex assessments


• create treatment plans


• prescribe medication

Specific Duties:

• Manage a caseload of DSNP Medicare members


• Diagnose treat and document monthly in EMR on all members


• Coding and documentation responsibilities


• Quality gap closure on all members


• Deliver Model of care

Contract/Shift Info:

• Hours: Monday through Friday 8-5pm


• Dress Code: Business casual


• Job Type: Contract – Temp to Perm
