Community Nursing Services is seeking an Occupational Therapist (OT) to provide occupational therapy services to home care patients in the comfort of their home, as well as in skilled nursing facility. OT will work in Brigham city, and Logan. This exciting position provides the opportunity to work with an experienced clinical team, in a very patient focused setting. The qualified candidate will have graduated from an accredited program and be licensed or eligible to be licensed in the state of Utah. Shifts are available for full-time, part-time and PRN positions.
Responsibilities include:
- Initial and ongoing assessments to determine level of functioning, including OASIS assessments at appropriate time points.
- Developing and revising the plan of care in consultation with the physician and other care-team members.
- Identifying barriers to desired outcomes.
- Goal setting.
- Providing therapeutic treatments in accordance with the plan of care.
- Evaluation of equipment needs to increase functional level.
- Preparing clinical and progress notes.
- Coordinating services in consultation with home health staff.
- Evaluating the effectiveness and outcomes of care.
- Supervising Therapy Assistant and HHAs as appropriate.
- Planning for discharge.
- Practice standard precautions.
- Participate in quality and performance improvement measures.
Requirements:
- Licensed occupational therapist
- Current CPR certification
- Utah driver’s license
- Reliable transportation
