Qualifications Required:
- Bachelor’s degree or Associate’s degree from an accredited college or university in Nursing and one year of professional nursing experience in home care.
- License to practice professional nursing as issued by the Alabama Board of Nursing.
- Valid Driver’s license
Job Duties:
- Provides patient care management to include coordination of care from admission to discharge.
- Performs, assigns and reviews nursing visit documentation.
- Documentation accurately reflects patient status.
- Provides all patient care according to the Nurse Practice Act and agency policy and procedures.
- Completes all patient care documentation and departmental documentation in a timely manner.
- Participates in an ongoing Subunit/LC Office QI program.
- Communicates professionally with internal and external customers, to include but not limited to, public health staff, hospitals, nursing homes, medical facilities, and/or physicians.
- Rotates on-call status with other staff nurses.
- Routinely meets weekly productivity goal for patient care visits.
- Responds to emergency and disaster assignments when called to duty.
