RN - CCMC Case Manager Job Opening

Genacta In Home Care is a Home Health Care/Personal Care Agency. We employ Personal Care Assistants & Direct Service Professionals to provide services to individuals experiencing physical illness & disabilities. Our clients range in age.

We are currently seeking a part-time RN (10-20 hrs/week) to oversee our CCMC (Children with Complex Medical Conditions) Case load. CCMC experience preferred.

Duties include;

*Monthly visits to CCMC Recipients

*Maintaining RN State Notes regarding visits with CCMC recipients

*Maintaining professional & timely communications with CCMC recipients, parents, guardians, care coordinators, & other Genacta employees

*Monitoring and overseeing Group Home Medications, Med Sheets, & Health Tracking

*Teaching Medication Administration training's to Group Home DSP's

Qualifications, education and experience required;

*High School Diploma or equivilant

*Current Nursing Certificate

*CPR/FA - Infant, Children, & Adult

*Two forms of Valid State ID

*Must be able to pass State & Federal Background Checks

*Valid State of Alaska Drivers Licenses & proof of State required vehicle insurance

*Negative TB Clearance

The RN will report directly to Genacta's Director of Operations, the Wasilla Office Manager, & the Group Home Supervisor.
