CARE COORDINATOR II & III- DELEGATED

JOB CATEGORY: ADMINISTRATION / QUALITY

POSITION SUMMARY: Under licensed supervision, the Care Coordinator II – Delegated works closely with the patient/family and the Interdisciplinary Care Team to proactively assess, plan, implement, monitor and evaluate options and services to effect an appropriate, individualized plan for the patient across the continuum of care. The process addresses the patient’s multiple environments of care and healthcare system needs. The position requires a focus customer service skills, knowledge of goal setting, and measuring outcomes to effectively ensure the patient has the most optimal outcomes provided in the most cost effective manner. Adheres to procedures and timelines as specified in the Managed Care Organization (MCO) contract and PMS procedures.

REQUIREMENTS:

EDUCATION AND/OR EXPERIENCE:

  • Graduate of an accredited nursing program (LPN or RN);

OR

  • Master’s degree in behavioral health or health care field.
  • Minimum one year clinical experience.
  • Good knowledge of personal computer and software for word processing and the ability to retrieve data from relevant computer systems.
  • Self-starter with strong verbal and written communication skills.
  • Highly organized with excellent follow-through skills.
  • Comfort with change and quality improvement processes.
  • Required to pursue ongoing education, certification, and/or self-development to remain current with industry standards and business objectives related to care coordination.

Preferred

  • Experience with Electronic Health Records.
  • Experience with Motivational Interviewing.
  • Knowledge of New Mexico and local community resources.

CARE COORDINATOR III - DELEGATED

CERTIFICATES, LICENSES, REGISTRATIONS:

  • Certified Case Manager (CCM) preferred.
  • Must obtain by date of hire and maintain current throughout employment:
  • RN license for New Mexico; OR
  • Independent license as a Social Work professional in New Mexico (e.g., LISW, LPCC, LCSW);
  • New Mexico Board of Nursing license with certification as a Nurse Practitioner or as a Clinical Nurse Specialist / Family Practice or Certification from the New Mexico Board of Medical Examiners.

CERTIFICATES, LICENSES, REGISTRATIONS:

  • Must obtain by date of hire and maintain current throughout employment:

o LPN or RN license in New Mexico; OR
o Non-independent license as Mental Health professional in New Mexico (e.g., LPC, LMHC, LMSW, LPAT)

  • Certified Case Manager (CCM) preferred.

SCREENING REQUIREMENTS:

  • This position requires successful completion of an initial post-offer of employment:
  • TB Test (and upon exposure in accordance with PMS Policies).
  • According to PMS Human Resources Policy 205.006, “Healthcare Personnel Testing & Immunization Requirements,” this position must comply with required testing and immunizations.Testing and immunizations including TB, Hepatitis B (consent or waiver), Tdap, MMR, and Varicella, will be available at the employee’s PMS health center. Influenza vaccines will be required according to dates set by the CDC and NMDOH.
  • Criminal Records Check.

DRIVING REQUIREMENTS:

  • May be required to use personal transportation to carry out job duties.
  • Must possess and maintain a valid Driver's License.
  • Must provide a current motor vehicle record.
  • Must provide current personal owner liability automobile insurance and must maintain the insurance coverage throughout the course of employment in this position.
  • Must submit to a motor vehicle record check on an as needed basis and maintain a satisfactory driving record that is within the limitations of PMS Human Resources Policy II.D.3, Automobile Usage, and applicable state and federal laws.
  • Must comply with PMS Policies on Automobile Usage, II.D.3, Drug Free Workplace, III.H and applicable state and federal laws.
  • May be required to complete transportation related trainings.
