Basic Function and Responsibility
Function as a clinical nurse expert and care coordinator for a specific patient population interfacing with administrative, clinical nursing staff, physicians and patients/families.

In this role, the nurse would coordinate the care of that patient from their initial contact of ocular oncology patients for the Department of Ophthalmology through surgery and follow up. This would include obtaining and reviewing outside records, arrange necessary testing or screening with appointments, and be a liaison to referring/outside providers. Coordinate care with other discipline including radiation oncology, surgical co management, anesthesia and other disciplines as needed. Provide education to patient and family regarding plan of care, resources, and assist with identifying financial resources/SW/support services if needed. Ensure equipment and specific plaque/radiation is arranged and coordinated for specific patient for surgery. Triage and follow up calls to pateints pre and post surgery to address needs or concerns.

Characteristic Duties and Responsibilities
  • Collaborate with nursing staff and other health care professionals in the development and review of standards and in planning, implementing and evaluating patient care for a specific patient population.
  • Provide direct nursing care, facilitate continuity of care and follow universal precautions.
  • Coordinate and facilitate patient care between multiple disciplines including home health agencies, referring physicians and other community support services.
  • Coordinate patient care to better utilize supportive services through patient visit and possible admission, pre-schedules testing, follows up with test results to both patient and physician, obtains medical history, coordinates follow up visit appointment, provides medication, rehabilitation teaching, etc.
  • Serve as a clinical resource or consultant for specific patient population.
  • Participate in development and implementation of managed care programs for a specific patient population.
  • Communicate with physicians and members of the health care team directing patient care.
  • Manage and support Telephone Triage.
  • Provide leadership for direct patient care programs in area of specialization collaborating with unit-based nurse management team(s).
  • Participate in decision-making process for financial management, operational budgetary decisions and cost containment strategies.
  • Participate in/provide leadership for divisional, departmental and/or intra institutional and community committees and projects.
  • Participate in the ongoing implementation of current regulatory standards in order to meet state and national agency accreditation.
  • Perform other special projects and tasks as assigned.
  • Design and implement patient/family health education activities for the patient population.
  • Serve as an educational and clinical resource to patient/family/staff.
  • Support the implementation and evaluation of staff education and orientation programs.
  • Facilitate the educational experience for health care students and monitor the care delivered.
  • Incorporate the role of patient care provider, advocate (patient, family, and health care professional), consultant, educator, researcher and quality manager.
  • Participate in the development and implementation of division/unit quality assessment and improvement programs.
  • Use relevant nursing research to improve clinical practice and investigate nursing care problems.
  • Participate in development and conduct of research activities related to area of specialty.
  • Responsible for maintaining clinical expertise consistent with patient population requirements and current nursing and medical practice.
  • Adhere to professional and legal standards of nursing practice.
  • Maintain confidentiality of patient and staff information.

Position Qualifications
Education Required
  • A Baccalaureate degree in Nursing
QUALIFICATIONS
  • Associates Degree and BSN by time of appointment
  • Current license to practice nursing in Iowa
  • Reasonable (1-3 years) nursing experience
  • Demonstrated clinical expertise in eye and/or tumor
  • Ability to anticipate needs, problems and potential issues and use judgment, foresight and problem-solving skills to troubleshoot, recommend and implement solutions.
  • Demonstrate careful attention to detail and accuracy, strong organizational skills and ability to handle multiple tasks quickly and effectively, proactively establish priorities and meet deadlines.
  • Excellent written and verbal communication skills required including ability to compose communications and grammatically correct documents in a concise, logical and organized manner.
  • Proficient in computer software applications (Microsoft Office Programs)

DESIRED QUALIFICATIONS

  • Reasonable (1-3 years) eye experience
  • Reasonable (1-3 years) tumor/oncology experience
  • CON (Certified Oncology Nurse) certification
