We are looking for a dynamic and passionate Social Worker-Case Management to join the Mountainside Team!

What You’ll Do

The Social Worker (MSW) will be responsible for providing direct clinical social work services to patients and their families in a multidisciplinary team approach.

The successful candidate will be able to:

• Perform assessment, planning implementation, facilitation and evaluation of discharge plans for patients requiring referral to another institution or community services

• Development of transitional plan, participate in discharge from transitional plan

• Monitor and identifies administrative issues that may affect reimbursement or increased length of stay

• Provide counseling as requested by patient/physician and referrals for counseling to community agencies for long term treatment

• Collaborate with Case Manager RN and nursing staff to facilitate plan of care related to discharge planning

• Maintain adequate and timely progress notes in each patient’s medical record by the end of the shift

• Obtain appropriate consents from patients and families/significant others

• Refer patient to appropriate entitlement programs/services

• Refer patient to appropriate alternative care facilities which is based on patient’s health care needs, finances, availability of resources and patient wishes

• Collaborate and communicates with case manager to reassess plan on an ongoing basis

• Perform any and all other duties as assigned

What You’ll Bring

• Master’s prepared or Bachelor’s degree in Social Work or related field
• 2+ years recent acute clinical experience or healthcare experience preferred
• Prior Case Management experience preferred
