Registered Nurse Case Manager Job Opening

Oasis Healthcare is seeking a Full-Time, Registered Nurse Case Manager to service the Tuscaloosa area. Candidate must reside within or live in reasonable distance from this area. This position includes a sign on bonus.

Hospice Registered Nurse duties include:

• Performing in-home or place of living evaluation and establishing an individualized care plan and medical record baseline for each patient.
• Collaborating with physicians and other agency staff to implement and coordinate the patient's plan of care.
• Guiding patients and their families to achieve peace of mind that enriches their lives as we help them accept the life/dying process.

• Employee Development: All new employees are oriented in the office as well as in the home utilizing a number of training techniques including shadowing, return demonstrations, competency assessments and hands-on training in patient’s homes. Requirements:

• Previous Hospice Registered Nurse Case Manager experience is required.
• Valid Alabama Registered Nurse License
• Valid Alabama Driver's License
• Proof of Car Insurance
• Great Clinical Assessment Skills
• Strong desire to serve others and to enrich lives
