Hours/Days/Shift: Day Shift

POSITION SUMMARY:

Provides patient/family care in accordance with hospice philosophy and delivery of care. Provides skilled nursing including interventions and documentation, supportive care and assessments for patients. Assists patient/families with external resources such as home health, financial concerns, counseling, etc. Educates and motivates patient/families regarding actions and behaviors towards self-care, wellbeing and self-empowerment.

KEY REPORTING RELATIONSHIPS:

Reports to Clinical Manager or Director of Clinical Services

ESSENTIAL DUTIES:

Coordinates skilled nursing care to patient as determined by patient’s condition, Plan of Care (POC), continuous assessment and monitoring of patient’s physical, emotional and spiritual care needs.

Develops care plan with patient/families, primary physician and IDT. Reviews and evaluates patients and POC in IDT meetings on a regularly scheduled basis.

Prepares, dispenses and charts medications, demonstrating basic and reasonable knowledge of the medications and their side effects.

Coordinates transfer of patients from home hospice to inpatient or to home hospice as appropriate.

Provides accurate and timely documentation in electronic medical record.

Assists in the orientation/training of new team members.

Participates in hospice related services such as community education, team training, staff meetings, board committees, in-services, and other sponsored functions.

Performs complex telephone interviews and triaging as appropriate.

Other duties and responsibilities as assigned.

Provides after-hours care to patient/family on a rotating basis and reports to appropriate team members.

Attends weekly team meetings and in-services on a regular basis and participates in continuing education and professional development.

POSITION QUALIFICATIONS:

Minimum Education: Graduate of an accredited school of Nursing.

Minimum Experience: One year experience in hospice/palliative care, medical/surgical, public health, oncology, acute care or home health. Three or more years’ experience as a nurse preferred.

Required License: Colorado RN license. Colorado state driver’s license and proof of auto insurance.
