Hospice Clinical Supervisor / Care Coordinator

Job Duties:
  • Field supervision of hospice staff
  • Assign patients and manage equitable territory assignments
  • Coordinate care for new hospice referrals assuring equipment is ready in the home and staff is coordinated to meet the patient at their home upon hospital discharge; meet with hospitalized patients and their families to discuss care coordination and provide emotional support
  • Conduct IDT and finalize IDT documentation
  • Orientation of new staff to hospice
  • Ongoing staff education in patient care, documentation, Medicare COPs, and levels of care
  • Conference with potential hospice patients and their families regarding hospice services
  • Participate in community functions to educate on hospice services
  • Active participation in all committees
  • Monitor and manage hospice expenses for appropriateness (DME & medications)
  • Hospice admissions as needed
  • Management of F2F schedules
  • Other duties as assigned
Qualifications:
  • 2 years experience in nursing supervision required
  • 3-5 years experience as a Hospice or home care case manager required, hospice preferred
