  • SUMMARY OF RESPONSIBILITY - Job Summary (general statement of purpose or objective of position)
    Demonstrates proficiency, autonomy and accountability in assuring patient care management needs are met from a well coordinated interdisciplinary and collaborative perspective with case manager and nursing supervisor. Maintains understanding of performance measurements. Takes leadership in Performance Improvement and Customer Service initiatives. Must be able to demonstrate the skills and knowledge necessary to provide intervention appropriate to the age, education level, and capabilities of the person served.
  • MINIMUM REQUIREMENTS - Knowledge, skills, abilities, license, registration, certification, education, and experience
    Bachelor's Degree in Social Work. Licensed in the state of Arkansas.
