We are looking for an experienced health care professional for our Quality Improvement program.

ESSENTIAL FUNCTIONS:

  • Uses quality criteria set by payors, government agencies, and national groups to determine current and ongoing quality targets to be achieved by Mid-State Health Center.
  • Develops programs, methods, and interventions for addressing quality concerns, reaching quality targets, and achieving improvements in process measures and clinical outcomes.
  • Responsible for quality reporting in accordance with guidelines set forth by, payors, government agencies, and other agencies/organizations by contract or agreement.
  • Responsible for the ongoing development, implementation, and coordination of population management strategies and effectiveness evaluation programs.
  • Serves as an interdepartmental resource on quality improvement activities.
  • Helps to coordinate programs related to health promotion, disease prevention, and chronic disease management using best-practice initiatives. Maintains positive interdepartmental working relationships for the purpose of streamlining operations and implementing quality program initiatives.
  • Maintains current knowledge of trends and issues in quality metrics and health analytics, population health and care management, health promotion/education, and patient engagement, and the patient-centered medical home model of primary care.
  • Engages staff in the organizational culture of quality and in program development and problem-solving.
  • Responsible for coordinating focused studies, audits, and pertinent committees.
  • Coordinates the development, maintenance, and utilization of statistical reports for quality improvement and reports information to the Quality Committee
  • Assists with special projects, such as grant applications.
  • Other duties as assigned.

The job holder must demonstrate current competencies applicable to the job position.

*BA/BS/BSN in health education, nursing or related field. Master’s degree preferred in Healthcare Administration (MHA) or Public Health (MPH).

EXPERIENCE: Minimum of two years quality management, nursing or health education. Experience in training and development is desirable. Successful management and leadership experience.

LICENSES: Valid state driver’s license and up to date auto insurance

KNOWLEDGE and ABILITIES:

  • Training and developing concepts and techniques.

  • Data interpretation.
  • Working independently and as part of the management team.
  • Planning, organizing, and implementing programs.
  • Performing highly complex and varied tasks requiring independent knowledge, its application to a variety of situations, as well as exercise of independent judgment.

PHYSICAL/MENTAL DEMANDS:

Work may require hand dexterity for office machine operation, stooping, bending, mobility to complete errands or deliveries, or sitting for extended periods of time. Occasionally lifts and carries items weighing up to 50 pounds. Requires corrected vision and hearing to normal range. Requires manual and finger dexterity; for things such as typing, using calculator, and eye-hand coordination.

Requires the ability to plan and perform complex work that involves new or constantly changing problems where there is little accepted method or procedure. Involves participation in the formulation and carrying out of policies, objectives and programs for major divisions or functions. Considerable ingenuity and exceptional judgment required to deal with factors not easily evaluated, interpret results and make decisions carrying a great deal of responsibility.
