Description:

Providence is calling a Population Health Resource Specialist (PMG Clinical Support Services, Day Shift, 1.0 FTE/40 hours per week) to St. Patrick Hospital in Missoula, Montana. We are a community of caregivers delivering every day on our Mission to provide compassionate care that is accessible for all – especially those who are poor and vulnerable.

In this position you will:

  • Work collaboratively with all providers and staff members, with a focus and emphasis on nursing and medical assistant staff members, to develop clinical support processes that ensure the delivery of efficient, compassionate and high quality care.
  • Work within a centralized team helping providers, clinic staff and management to effectively, efficiently and accurately fulfill the duties and responsibilities of the centralized services department in support of our Patient Centered Medical Home objectives.
  • Perform assigned tasks and activities under the direction of the Practice Manager.
  • Be responsible for obtaining pertinent patient information for referrals, prior authorizations or updating patient medical records.
  • Follow up on the patient appointments and notes, and ensure it is received by the referring provider.

Qualifications:

Required qualifications for this position include:

  • High School graduate

Preferred qualifications for this position include:

  • One to two years of referral tracking and coordination experience
