Hospice RN Job Opening

Evaluates and admits patients to Hospice Care, develops plan of care and provides care to patient according to plan

Provides end of life care and coordination at time of death and bereavement support following death

Acts as case manager making necessary adjustments in patient's plan of care related to symptom changes and changes in care needs

Acts as Liaison for pt/caregiver with MD; Community health care partners and other hospice team members.

Completes all documentation related to pt contact; assessments and IDT meetings according to policy

Provides on call coverage for both Home care and Hospice as assigned

Makes necessary Home Care visits as assigned

Demonstrates and encourages team behavior and exceptional patient/guest experiences.

Upholds and promotes patient safety and quality.
