Intake Referral Coordinator Job Opening

Kershaw Health has an opportunity for a Intake Referral Coordinator in our new Geriatric Psychiatry Unit. We specialize in elder care issues, including
behavioral and health needs, medication management, and conditions specific to those age 65 years and older including Alzheimer’s disease, depression, anxiety, as well as other mental illnesses or behavioral health concerns.The Intake Referral Coordinator Reviews incoming referrals and presents relevant information to the provider on-call.
The Intake Referral Coordinator is a point of contact for referral sources and provides seamless coordination between the referral sources and the unit.
Conduct pre-certification request for medical necessity inpatient level of care criteria and verifies insurance.
Assist as necessary with alternative placement recommendations for outside referrals when patients are not appropriate for admission.
Conduct concurrent clinical reviews for continued stay as backup coverage for utilization review as needed.
Provide social work duties (psychosocial assessments, treatment plan formulation, discharge planning, group therapy) as back-up coverage as needed.

Must have a bachelor's degree in social work.

Preferable experience in case management, psychiatry, and or utilization review.
