Assessment & Referral Nurse (LPN)

Purpose

To provide quality nursing care while admitting patients into Lake Pines Hospital. Supports and assists the Medical Director and Director of Nursing in maintaining appropriate standards of care and meeting all applicable state and federal laws.

Essential Duties and Responsibilities

• Get report from referral source
• Review legal status forms for accuracy and completeness.
• Conduct nursing head to toe psych admission assessment, including skin assessment; and suicide/ risk assessment to include protective factors
• complete Admission Trigger Screen for services
• complete AIMS and other scoring tools as indicated
• complete an admit note
• go over consents and rights - if they fail to sign make sure it is documented on forms and re request signatures daily and note if refused each time
• complete a care plan
• flag precautions for room and review armband for appropriate precautions and ensure Q15 has correct precautions noted
• assign patient to PCL and room placement based on risk assessment
• ensure patient belongings properly inventoried and documented and stored
• offer patient formally voluntary if on PEC unless physician states otherwise
• complete admit d/c log with patient information
• Place patient in suicide risk beds if they score high on SRA and/or risk assessment section of nursing assessment
• assign patient code and enter patient information onto the intake log
• add patient to midnight census
