POSITION PURPOSE
Under the direction of the Nurse Manager/Designee, the Wellness Assistant will provide supportive daily care and services in accordance with the resident care plan. Assists in the administration of medications as ordered by the attending physician under the direction of the Nurse Manager / Designee and in accordance with established nursing standards, policies, procedures and practices of the community and all state regulatory requirements. As directed by the Nurse Manager/Designee, the Wellness Assistant will proactively identify, develop and execute solutions to enhance the resident living experience.

ESSENTIAL FUNCTIONS

Create and maintain an atmosphere of warmth, propagating a calm environment throughout the community. Execute assignments in a culture that is shared and collaborative across all divisions within TSLC. Follow work assignments, and/or work schedules in completing and performing assigned tasks. Enters information and documentation accurately in Care Tracker or paper chart in accordance with company policy, practice and standards. Assist residents with daily living functions such as dental care, feeding, bathing, dressing/undressing, nail care, hair care, and shaving according to expressed preference. Assist with lifting, turning, moving, positioning, and transferring residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc. Escort residents to and from meals, activities etc., when needed. Assist in the dining process, serving residents their meals. Report changes in the residents eating habits to the supervisor, as well as performing after meal care cleaning resident’s hands, faces and clothing as needed. Develop and maintain a good working rapport with other departments within the community to assure that daily activities can be performed without interruptions. Check residents routinely to ensure that they’re safe and that their personal needs are met. Observes and reports to supervisor the residents physical and emotional responses to service treatments and medications or any changes in resident condition. Answer resident calls promptly to determine need and help when possible. Provides for the physical needs of the resident when indicated. Requests supervisor’s assistance with resident problem. Anticipate the needs of residents and families, going “above and beyond” routinely to provide a comfortable environment and home atmosphere. Follows community assignment sheet as established by supervisor. Cleans equipment, closets, drawers, beds and other resident living spaces as needed. Leaves the immediate resident environment in a clean and safe manner. Demonstrates proper use of resident care equipment. Responsible for administration of medications as delegated by the supervisor. Ensure safety in a home environment. Report all hazardous conditions and equipment to the supervisor. Follow established safety precautions in the performance of all duties. Respect resident rights, including maintaining the confidentiality of all resident care information, knocking before entering a resident's room, honoring the residents' personal and property rights, informing the resident when it is necessary to move his/her personal possessions and reporting complaints of missing property to Supervisor. Attend and participate in scheduled training, in-service training, mandatory annual in-service training and educational classes as required/needed. Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with the community policies and procedures.
MINIMUM QUALIFACTIONS
High School Diploma or equivalent. Must be at least 18 years of age to work in the AFC section of the Village. Six months experience as a Wellness Assistant or Nurses Aide preferred. Previous experience working in assisted living or with the geriatric population preferred. Must possess superior customer service skills and professionalism. Must be able to pass the written in-house medication training program and/or written exam and clinical competency demonstration. Must possess strong verbal and written communication skills. Ability to adapt to ongoing change and work in a fast-paced, customer-driven environment. Possesses interpersonal and relationship building skills to work in a cross-functional team as well as the ability to work with residents based on their cognitive ability. Self-starter, highly motivated with a high productivity level. Possesses a high degree of personal accountability, responsibility and strong decision making abilities. Must possess the ability to plan and organize. Basic to intermediate proficiency with Microsoft product suite (MS Word, Excel, Power Point, etc.); basic knowledge and experience with electronic mail and calendaring system in PC LAN environment. Ability to type with speed and accuracy. Position requires minimal travel (less then 10%) within the Community’s geographic region and/or to home office in Livonia, MI.
PHYSICAL AND MENTAL REQUIRMENTS AND WORKING CONDITIONS
Must be able to adapt to frequently changing work parameters. Must be able to see, hear and smell or use prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met. Primarily works inside with frequent kneeling, crouching, reaching, sitting, walking, bending and lifting. Must be able to push, pull, move and or lift a minimum of 25 pounds to a minimum height of 3 ½ feet and be able to push, pull, move and/or carry such weight a minimum distance of 10 feet. May be necessary to assist in the evacuation of residents in emergency situations.

Must possess the above ability with or without the use of prosthetics that will enable adequate functionality so that the requirements of this position can be fully met.
Subject to exposure to noise, infectious waste, diseases, conditions, etc. including TB, HIV, HEP B viruses. May be subject to the handling of and exposure to hazardous chemicals. Must be able to speak, read and write the English language in an understandable manner. Must be able to relate to and work with ill, disabled, elderly, emotionally upset and at times hostile individuals within the community. Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public. Must not pose a direct threat to the health or safety of other individuals in the work place. Must be able to cope with the mental and emotional stress of the position. Must meet the general health requirements set forth by the policies of this community, which include a medical and physical examination. May be required to work beyond normal working hours, on weekends and other positions temporarily when necessary and may be subject to call back during emergency conditions. May be required to work on shifts other then those, which originally hired.
