Since 1946 Easterseals Hawaii has been providing services to the their local communities. We are seeking individuals to join our Maui Home and Community Based Services as a Case Coordinator. Come join our team as we continue our expansion as a CARF (Commission on Accreditation of Rehabilitation Facilities) and non-profit organization throughout Hawaii.

Case Coordinator Responsibilities:

  • Maintain a caseload of 15-20 participants in Adult Day Health and Support Services
  • Assess participants' current level of functioning and provide assistance and coaching to individuals with developmental disabilities to meet goals and outcomes that increase their independence, develop natural supports and friendships, and the ability to contribute to society
  • Coach these individuals in a variety of life skills, including socialization, communication, and money management.
  • Supervise Direct Support Workers, Community Based Providers, Program Assistants, and Activity Coordinators to ensure Waiver/CARF compliance
  • Assist Program Manager in concluding staff orientation, training, and performance evaluations

Case Coordinator Requirements:

  • Bachelor's degree
  • 1 year of experience in working with people with Developmental Disabilities
  • Valid Driver's License, abstract, vehicle insurance, and registration
  • Current First Aid and CPR certification
  • Proof of annual tuberculosis clearance
