Summary

JOB TITLE: Clinical Care Coordinator

GENERAL PURPOSE:

The Clinical Care Coordinator helps to enhance the quality of patient management, maximize satisfaction and promote cost effectiveness. Duties include assessing, planning, implementing, monitoring and evaluating actions required to meet the client’s health and human services needed, including handling case assignments, draft service plans, review case progress and determine case closure.

QUALIFICATIONS:

  • Graduate of a school of nursing program and currently licensed as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) by state board of nursing or 1 year of experience as a Medical Assistant.
  • Proven working experience in case management, including as a nurse, medical, mental health care manager or a related job.
  • Excellent knowledge of case management principles, healthcare management and reimbursements.
  • Effective communication skills.
  • Excellent organizational and time management skills.
  • Familiarity with professional and technical emerging knowledge.
  • Problem solving skills and ability to multi-task.
  • Current active license, certification or registration in good standing.
  • Must be a licensed driver with an automobile that is insured in accordance with state/or Company requirements and is in good working order.
  • Complies with acceptable professional standards and practice.
  • Demonstrates good communication and management skills.
  • Must be able to relate positively and favorably to the patients, families and outside agencies and to work cooperatively with other employees.
  • Must be capable of performing the essential job functions of this position with or without accommodations.

ESSENTIAL JOB FUNCTIONS:


  • Answer phones and relay messages to clinicians or Senior Management.
  • Answer clinical related, scheduling, and patient questions and concerns.
  • Enter historical summary on referrals received so that clinical staff will have patient information in EHR system.
  • Communicate with clinical staff regarding patient medical history, patient scheduling.
  • Contact patients to ensure consent to services before clinical visit.
  • Contact physicians’ offices to acknowledge referrals and status.
  • Coordinate MD order process through Echo Sign.
  • Place signed orders on chart and fax to appropriate home health or other agency.
  • Assist with scheduling.
  • Order durable medical equipment (DMEs) and lab supplies for clinicians.
  • Track lab results and put on chart/notify NP as necessary.
  • Enter orders into VuePoint for portable radiology tests as clinician orders.
  • Obtain medical records for the clinicians as needed.
  • Refer patients to clinics and ensure appointment dates provided to patient.
  • Monitor faxes and add relevant patient data to files.
  • Assist with transportation set up as needed.
  • Assist billing with clinical information.
  • Scan patient information into charts.
  • Coordinate communication with MD offices regarding clinician questions, patient status.
  • Monitor/assign new admits to the clinicians per patient flow/patient area.
  • Use analytical skills to solve wide range of problems on a daily basis.
  • Case management for the clinicians.
