Case Manager / Social Worker
Full Time (Monday - Friday; 1st shift)
Health Partners of Western Ohio is searching for a Case Manager/ Social Worker for our Kenton Community Health Center. This position is responsible for providing case management and outreach services to patients of special populations or with health care needs that are not available within the scope of services of HPWO.
DUTIES AND RESPONSIBILITIES:
  • Promotes effective and appropriate use of integrated health services at HPWO as part of a “Patient Centered Medical Home” (PCMH) model of care.
  • Assists with the coordination of care for patients as directed by the healthcare providers.
  • Completes assessments of patient/family needs with respect to physical, psychosocial, educational and all other needs.
  • Coordinates and facilitates a medically appropriate plan of care with the patient as directed by the provider.
  • Coordinates efforts with the Behavioral Health Consultants to ensure duplication of efforts do not occur and information is shared.
  • Identify patients meeting criteria for case management (i.e. high-risk, problem-prone patients) and implement/oversee case management services.
  • Serves as a resource for the health team, patient, and family.
QUALIFICATIONS:
  • Bachelor’s Degree in social work, sociology or psychology or related field, or related experience.
  • Licensed Social Worker (LSW) in State of Ohio preferred.
  • Minimum of three years of experience in a mental health or medical field with documented clinical course work and field experience.
  • Demonstrate the ability to identify planning related to patient learning needs and to assess patient’s knowledge, skill level and readiness for learning.
  • Excellent oral and written communication skills required.
  • Ability to work with persons form a wide diversity of social, ethnic and economic backgrounds is necessary.
  • Must be comfortable with presentations in the community.
  • Ability to demonstrate positive customer service skills.
