Responsibilities

Facilitates resource management activities for inpatients based on identified patients. Coordinates patient�s stay from admission to discharge through collaboration with physicians, nursing staff and other healthcare professionals to enhance continuity of patient care, promote cost effective utilization of resources and enhance patient, nurse and physician satisfaction.

Qualifications

Minimum Knowledge, Skills, Experience Required
Education: Require BS degree in Social Work from an accredited college or university and current Alabama icensure.
