POSITION PURPOSE
Under direct supervision, assists in carrying out the phases of the nursing process as appropriately delegated by the Registered Nurse/Provider. Provides nursing care to assigned patients by utilizing the nursing process and meeting the established standards within the clinic. Is accountable for assigned patients, demonstrates understanding of nursing and its issues and is an advocate for patient/families and nursing. Adheres to the principles of Service Excellence Guiding Behaviors and the philosophy and mission of Mercy in all aspects of job performance.

ESSENTIAL JOB FUNCTIONAL COMPETENCIES:
1.
Demonstrates the knowledge and skills necessary to provide service/care appropriate to the age of the patients served, in accordance with Mercy standards.
2.
Utilizing the nursing process assists the registered nurse/provider in providing patient care.
3.
Demonstrates ability to identify physical, psychological, social and spiritual needs of the patient and appropriately reports to the registered nurse/provider.
4.
Promotes and maintains a safe and calm environment for complex patients by ensuring appropriate supplies, equipment, and necessary services.
5.
Identifies actual and potential risk to patients and/or families and initiates appropriate action to minimize risk.
6.
Administers medications and treatments/procedures as directed, and as appropriate for the patient’s medical care, in collaboration with the multi disciplinary team.
7.
Utilizes internal and community resources in planning care and initiating referrals.
8.
Initiates appropriate patient/family education following assessment of learning and discharge needs.
9.
Documents nursing intervention and patient/family response accurately, legibly, and in compliance with clinic policies.
10.
Appropriately delegates tasks/procedures to other team members who are prepared/qualified to perform.
11.
Demonstrates the skill level requirement for a Licensed Practical Nurse as evidenced by competent performance and completion of the skills checklist.
12.
Contributes to the learning experience of students and staff.
13.
Uses effective communication principles to coordinate care with other health care providers.
14.
Actively participates in clinic meetings, committees, conferences, and in-services.
15.
Recognizes and communicates issues/occurrences utilizing established lines of authority, and assists in identifying ways to resolve variances or unacceptable outcomes.
16.
Is knowledgeable of and actively participates in improving patient safety and reducing risk to patients. Complies with Safety and Infection Control & Procedure.
17.
Knows, understands, incorporates, and demonstrates the Mercy and Trinity Health Mission, Vision, and Values in behaviors, practices, and decisions.
18.

MINIMUM QUALIFICATIONS
- Must be a licensed practical nurse maintaining current state licensure. A minimum of one to two years nursing experience preferred.
- Basic Life Support Certification (BCLS) is required unless doctor’s release is provided.
- Is able to communicate effectively with all members of the health care team.
- Must have a general working knowledge of computers and department specific software (i.e. Healthstream, internet, etc.)
- Must be comfortable operating in a collaborative, shared leadership environment.
- Must possess a personal presence that is characterized by a sense of honesty, integrity, and caring with the ability to inspire and motivate others to promote the philosophy, mission, vision, goals, and values of Mercy and Trinity Health.

PHYSICAL AND MENTAL REQUIREMENTS AND WORKING CONDITIONS
- Must successfully pass employment physical examination and must adhere to applicable organizational requirements on an annual basis for Employee Health Directives.
- Must possess the ability to comply with Mercy and Trinity Health policies and procedures.
- Has the physical stamina to meet the requirements of this job description, including frequent walking, bending, and standing.
- Able to lift or assist in lifting, transporting, etc. of patients or equipment, sometimes in excess of 50 pounds.
- Knowledge of body mechanics necessary.
- Able to function effectively under stressful situations.
- Must understand and accept the possibility of exposure to environmental elements, internal and external, which may affect the work place, such as infectious disease, chemicals and or chemical fumes, dust, noise, physical injury form an out of control patient, and adverse effects of inclement weather which may occur.
