We focus on caring for those affected by HIV, Hep C and other sexually transmitted diseases and helping to improve their lives and focus on stopping the spread of sexually transmitted diseases through prevention, public awareness, education and advocacy.
Position Overview:
Case Managers provide key assistance in four areas for the clients; access to an HIV specialist, HIV medicines, food and housing subsidy assistance. They work both independently and as a team with the other Case Managers to offer individualized care for each client, relying on their network of care providers and one another for additional expertise to help clients achieve improved medical outcomes. They meet regularly with clients one-on-one to conduct comprehensive assessments of individual need, and to provide assistance and advocacy regarding medical, social, community, financial and other needed services. Their careful oversight ensures that clients can more easily access medical care, substance abuse services, oral health care, mental health services, case management services and support services including transportation, housing, and public benefits.
Our Case Managers work closely with the client’s physician and with clinics to help maintain the records of each client. They will monitor each of the clients and make sure they maintain adherence to their medical needs by face to face meetings, phone calls and semi-annual evaluations.
All MAF Case Managers provide comprehensive, integrative services that are specifically targeted to address the changing, challenging and complex needs of individuals living with HIV/AIDS in Maui County, a rural region with relatively few opportunities to access specialized, urban health care.
Qualifications:
EDUCATION: Bachelor's degree or higher level education. Experience, knowledge and ability may be substituted for education qualifications. RN, LPN or MSW preferred but not a requirement.
EXPERIENCE: Experience providing community based social or health care services.
KNOWLEDGE AND ABILITY:
Knowledge of: Basic concepts of case management; social, health and medical factors affecting functioning of HIV infected individuals and their families, friends and significant others; addiction factors; community resources; financial assistance programs; and laws/regulations regarding health services, privacy and consent issues.
Ability to analyze the strengths and challenges of clients; develop sound, sustaining work relationships with clients, families, professional people and co-workers. Maintain an objective and emotionally stable attitude in dealing with varying types of situations and personalities. Obtain and accurately input/type client “notes” in the computer. Able to set clear, solid boundaries and limits when working with individuals with substance and mental health challenges. Able to work in close environments with individuals who sometimes may have chalenges with hygiene practices.
Job Requirements:
Ability to communicate effectively orally and in writing.
Type 35 words or more per minute.
Demonstrated ability to cultivate and maintain positive working relationships.
High level of proficiency with Microsoft Office software (Word, Excel, Publisher).
Experience with using a database mandatory (entering and exporting data).
Bachelor’s Degree strongly preferred.
Valid TB clearance (provided by MAF).
Traffic Abstract (provided by MAF).
Pass background check.
Must have own vehicle and a current driver's license and insurance.
Ability to lift 30 pounds, stand, bend, and sit.
References required.
Position Status: Full Time – 8 hours a day on Monday through Friday 8:30-4:30
