As a part of the OptumCare network, ProHealth Physicians is seeking a full time Adult/Geriatric Primary Care provider to join our team in West Hartford, CT. Primary Responsibilities:

  • Identify diagnoses to be used in care management and active medical management in the furtherance of treatment
  • Formulate a list of current and past medical conditions using clinical knowledge and judgment and the findings of your assessment

Required Qualifications:
  • Active, unencumbered CT License or ability to obtain
  • Board certification or ability to obtain
Preferred Qualifications:
  • 1+ year experience in primary care
  • Excellent administrative and organizational skills and the ability to effectively communicate colleagues, patients and families
