• Tigard, OregonThe Luke Dorf West QMHA Case Manager provides community and clinic based interventions designed to stabilize individuals on the Luke Dorf West clinical team in Washington County. The case manager works as part of the Luke Dorf West clinical team to implement, monitor, and document services to individuals enrolled on this team. Case Managers follows procedures, outcome goals, and reporting requirements as outlined by Luke-Dorf's contracts with Washington County. Services are both clinic and community based and emphasize making use of natural community supports such as: DHS, SSA, MH Court, family, friends, landlords, AFH support staff, employers etc. Case Manager will be expected to go to individuals home, as well as transport individuals to and from appointments and advocate for consumers.
Accountabilities:
Case Management
Luke Dorf West QMHA Case Manager provides services as outlined on the treatment plan for individuals who may benefit from supplemental QMHA support in addition to their QMHP. The team members provide comprehensive wrap around services to assist individuals to meet their basic needs, accomplish goals as outlined in the treatment plan, attain the highest level of independence possible, and continuously strive for mental health stability.
General Case Management Duties Include
- Assist consumer in connecting to resources needed to achieve goals set in the individual service and support plan.
- Monitor, coordinate, and document all agency services.
- Knowledge of TIER and ability to concurrently document client interactions
- Develop relationships with providers and stakeholders to facilitate service provision
- Have regular, frequent contact with consumers.
- Monitor medication distribution and assist with prescriber visits.
- Coordinate with money management services, and intervene as necessary with landlords and other community stakeholders to ensure residential stability and fiscal responsibility of consumers.
- Transport individuals to and from: DHS, SSA, MD, prescriber visits, therapy, etc.
- Advocate for consumer
Skills Training
- Collaborates closely with the clinic QMHPs to incorporate into each resident’s clinical master treatment plan the appropriate interventions to be administered by the QMHA Biller.
- Assist residents with skills training related to individual ADL and IADL goals dependent upon their individual Service Plans.
- Teach recovery and symptom management strategies to prevent relapse both of mental health symptoms and substance use
- Lead therapeutic groups to help participants manage mental illness, cope with anxiety and depression, anger, ADL and IADL management, and assertive conflict resolution
- Facilitate integration with low cost community resources
- Document all meaningful interactions with clients during each shift according to policy and Oregon Administrative Rules.
- Adhere to Confidentiality, Professional Ethics, and Dual Relationships per policy and Administrative Rule
Documentation
- Document all client contact within 24 hours of service delivery in Electronic Health Record/TIER.
- Utilize the ACORN website when appropriate to provide outcome-informed care.
Crisis Intervention
- Work proactively with individual to avoid use of crisis services.
- Complete all required documentation
- Coordinate respite referrals and entry’s as well as oversee respite stay.
- Observe hospital contact and service conclusion planning policy.
- Minimize number of days spent using crisis/respite services intensive by remaining closely involved in service conclusion planning.
Team Configuration and Meetings
The team will coordinate daily and meets for 1-2 hour weekly team meetings to staff cases and plan services. Meetings are clinically focused and function as group supervision.
Supervision
Luke Dorf West QMHA Service Coordinator Aide will report to the Luke Dorf West Program Manager.
Other General Expectations:
- Attend and positively participate in required meetings.
- Understand and implement Evidenced Based Practices in all work completed with individuals
- Pursue ongoing development of skills and competencies, including those required to maintain QMHA credential.
- Follow all corporate polices, procedures, and protocols.
- Maintain current Oregon Driver’s License with ability to transport consumers.
- Follow through on all directives and instructions.
- Maintain security of all corporate interests.
- Maintain CPR and First Aid Certificate.
- Maintain QMHA certification
Education/Certificates/Licenses/Registration:
- Qualified Mental Health Associate (QMHA) with a Bachelors Degree in social work or related field is required.
Experience/Qualifications:
- A minimum of two years experience in social work or counseling field required.
- Applicant must have prior experience providing QMHA level individual case management services and skills training groups.
- Applicant must possess the skills necessary to work independently.
- Detailed understanding of psychiatric medications and their use, administrative rules and other applicable laws and regulations, and thorough understanding of acceptable clinical practices and guidelines.
- Understanding of the DSM and the ability to understand mental health assessments and implement initial individual service and support plan interventions.
- Knowledge of and ability to utilize Motivational Interviewing.
- Knowledge and understanding of how to coordinate with community stakeholders, specifically local hospitals, ENCC’s with Washington County, and Washington County AFH providers.
Mental Requirements:
Extensive mental activities required. Examples to include but not limited to the use of educated judgment, communicate thought clearly, maintain calm demeanor in crisis situations, be an empathetic and effective listener, and flexible analytical and interpreting skills. Ability to motivate others and remain consistently enthusiastic.
