NeuroRestorative is seeking a Case Manager to meet and work with individuals to gather information and assess each client/consumer’s needs to include the assessment of the cost of care. The Case Manager may prepare a plan of care, and identify and implement services to meet an individual’s needs.
Qualifications:
Requirements:
  • Bachelors degree in Human Services or equivalent in education and experience required
  • In addition, one year of related work experience and knowledge of case management
  • Other education and experience plus licensure as required by state
