Care Managers are required to assess and evaluate member's needs to achieve the most advantageous wellness state by guiding member's and families toward resources appropriate for aging in the home. Care Managers work assignments vary and require independent decision making.

Responsibilities:

Care Managers will be required to utilize techniques to manage a member's physical environmental, and psycho-social health issues. Identify and resolve barriers that hinder effective care. Ensure patient is progressing towards desired outcomes by continuously monitoring patient care. May create member care plans. Must be well organized and provide excellent documentation. Applicants must be capable of making good independent decisions and follow all established guidelines/procedures.

Required Qualifications:

*Registered Nurse with 2 years of experience of in home care/care management

*Experience working with adults

*Knowledge of community health, social service agencies, and community resources

*Ability to travel to member's home within 50 to 60 miles

*Exceptional communication skills with the ability to quickly build rapport

*Ability to use a variety of electronic applications and software programs

*Must be able to pass a satisfactory background check

*Valid driver's license, car insurance, and access to an automobile

Preferred Qualifications:

.Registered Nurse

.2.5 years of in home assessment and care manager experience
