Mitchell-Hollingsworth Nursing and Rehabilitation Center, LLC is looking for a Clinical Admissions for our Admissions Department. This position involves meeting and evaluating prospective patients or resident's from hospitals, long term care facilities, homes, assisted living facilities, etc. for admission to Summit Rehab and our Long Term Care Units following the facility Admission process.

  • Evaluate new patients for Summit Rehab
  • Evaluate new residents for Long Term Care
  • Follow the admission process including gathering and evaluating background information, insurance information, meeting with the families, etc. to meet patient/resident needs
  • Evaluate the patient/resident level information
  • Check physician's orders
  • Meet with families of patients/residents
  • Work with Case Managers, Social Workers, Nurses, and Physicians at hospitals, nursing homes, physician's offices, home health offices, hospices, etc. to complete admission process
  • Follow physician's orders
  • Notify facility of equipment or supplies needed
  • Notify facility of any changes in admission status
  • Work with other Clinical Liaison out in the community to promote organization
  • Attend area functions and events as needed
  • Other duties as assigned by Administration
