SOCIAL WORK CASE MANAGER
Jewish Family Services of Greater Hartford
• West Hartford, ConnecticutThe Case Manager (CM) provides outreach, initial assessment, advocacy, case planning, service coordination, and case management for clients referred to the VOCA (Victims of Crime Act) program. Clients have been victims of crime either recently or in their past and need services to help them make a full recovery, usually from trauma or violence. You will be based in West Hartford, in the Hartford Region of a statewide program.
EDUCATION: BA/BS in a human service field with at least two (2) years relevant experience, Associates or equivalent with over five (5) years of directly relevant experience may be considered in lieu of BA/BS
We are looking for:
- Knowledge of VOCA services and systems in CT
- Significant knowledge of community-based services for victims of crime
- Ability to work with culturally and linguistically diverse families
- Ability to work independently in a home-based program
- Computer literacy in Windows and use of mobile devices, experience with ETO a plus, excellent documentation
- TEAMWORK FLEXIBILITY SOME NONTRADITIONAL HOURS
This intensive, client-centered program will require your skills for outreach, relationship building, assessment, goal setting, advocacy and more.
