Job Summary

**Hiring for Maui or Kauai Office

Work with Case Manager-Service Coordinators to assist them in providing care coordination for HMSA QUEST Integration/Akamai Advantage Dual Care eligible members. Service Coordinator Assistants will work collaboratively with the Case Manager-Service Coordinator to build relationships among members, providers, community agencies and HMSA in order to ensure members receive timely and integrated access to services across the continuum of care. Assist Case Manager-Service Coordinators in carrying out the service plans to ensure that the members are getting what they need timely

Minimum Qualifications

  • Associate's degree and two years related work experience; or equivalent combination of education and work experience
  • Effective verbal and written communication skills
  • Excellent interpersonal, problem-solving, and organizational skills
  • Effectively perform detailed research and create reports on the findings
  • Exhibit cultural sensitivity to people of various ethnic origins and socioeconomic backgrounds.
  • Effectively solve practical problems and deal with a variety of unique situations where only limited standardization exists

Duties and Responsibilities

**Hiring for Maui or Kauai Office
  • Prepare administrative work for the Service Coordinators to include but not limited to preparing HFA's, inputting 1147's, completing the call cadence to enroll members in the program, utilizing HMSA tools to identifying any gaps in care.
  • Ensuring that services are initiated timely to include but not limited to assisting the Service Coordinator in monitoring service plan goals, follow up with community referrals, monitoring expiration of service authorizations, preparing pre-authorization form to request services, assist members or providers with any issues or concerns about service delivery.
  • Assist members in following up with appointments with providers and finding specialists, especially for complex members. May require going in the field to attend appointments with Service Coordinators as needed.
  • Document all interactions in regards to members in service coordination in appropriate electronic systems.
  • Other duties as assigned.
