Join Alzheimer’s Community Care as a Case Manager serving Martin & St. Lucie counties. The Case Manager is a professional who administers social work skills that contribute to the emotional well-being and accessibility of supportive services for families, caregivers, or patients of Alzheimer’s disease and/or other conditions resulting in dementia.
To a family in need the Case Manager is much more than a gatekeeper to funding, they are their advocate and support system. Our Case Managers work with families with financial needs to determine their eligibility, so Alzheimer’s patients can remain in the community. Whether this funding is provided by the state, local government entities or through private funds secured by Alzheimer's Community Care, these funds are critical to protect the well-being of the patients and caregivers.
The successful candidate will possess excellent verbal, written and presentation skills; the ability to prioritize work schedule to successfully meet established deadlines and goals; excellent time management skills; competent in Microsoft Office suite products (Work, Excel, Outlook, PowerPoint); and work independently and as part of a team.
Additional requirements for the position are: possesses knowledge and ability to provide comprehensive care management, including knowledge of community and home health services, public health science, and the long term care continuum of community-based, home, and institutional services, and comfortable with public speaking and community teaching. The successful candidate must be able to enhance the image of the organization in person, by telephone, and in written communications.
Bachelor’s degree is required in social work, psychology, or related social service field. A minimum two years’ experience of direct case management experience in a long-term care environment, either community-based or residential. A Master’s degree may be substituted in lieu of the minimum experience requirement. Dependable transportation is a must to complete the 12 to 15 home visits a month.
The position is based in Stuart and hours are 8am to 5pm Monday through Friday.
