General Summary:

Our Team is looking for a Case Manager II for the Adult Outpatient department at Greater Lakes Mental Healthcare.

The Case Manager II is an entry level position that naturally progresses into a Therapist I role. The Case Manager II will assist adults at Greater Lakes’ outpatient services in their journey toward their personal recovery and wellness.

The Case Manager II would provide individualized, person-centered clinical case management services. Including:

  • Counseling

  • Psycho-education

  • Leading group sessions

  • Educational/vocational support

  • Housing assistance

  • Crisis stabilization

  • Assistance with medical, community and natural resources.

The Case Manager II will also partner with clients, their families, and significant others in obtaining, maintaining, and using appropriate mental health/community resources.

Additional goals would be providing counseling to help clients re-establish self-identity, sense of purpose and meaning in their lives, by developing plans for work, school, and/or volunteering, and making community connections.




RESPONSIBILITIES
  • Use effective clinical case management skills to assure appropriate client-centered care.
  • Complete all clinical and administrative documentation in a timely, accurate manner.

  • Manage all clinical and documentation functions in ways that meet department productivity goals.
  • Provides secondary and/or Optional CM2-Adult functions effectively.

QUALIFICATIONS
  • A.A. degree in a mental health related field plus two years of experience working with severely and persistently mentally ill; or equivalent combination of education and experience.
  • Knowledge and understanding of mental disorders
  • Thorough understanding of and commitment to the recovery model.
  • Familiarity with the DSM-V and competency in accurate diagnoses.
  • Knowledge of the major components of a mental status examination and competency in performing on-going assessments.
  • Ability to implement a continuum of care for consumers based on consumer need and level of functioning.
  • Ability to develop and implement recovery plans.
  • Basic knowledge of psychotropic medications with the ability to recognize side-effects.
  • Basic awareness of substance abuse/dependence issues.
  • Thorough knowledge of community resources and the ability to develop appropriate links for consumers.
