  • JOB SUMMARY

    Case Management includes the activities guided by a client’s treatment plan that have been identified by the primary clinician, which brings services, agencies, resources, and people together within a planned framework of action toward the achievement of established treatment goals, including wrap around services. The Case Manager works under the Supervision of the Mental Health Clinical Supervisor and/or the Clinical Care Coordinator.

    Case Management activities are guided by the client’s treatment plan which brings services, agencies, resources, and people together within a planned framework of action toward the achievement of established treatment goals.

    Case management activities include, but are not limited to:

    • Linkage
    • Monitoring/Follow-up
    • Referral
    • Advocacy
    • Crisis Intervention
  • PRIMARY JOB DUTIES
    • Assist the primary clinician in implementing treatment plan goals that requires assistance from the Case Manager.
    • Implements that portion of the client’s treatment plan which assists the client and family in gaining access to needed medical, social, educational, and community services. Communicate effectively with clinicians and provider to ensure activities associated with the client’s treatment plan are appropriate and are meeting the intent of the treatment plan.
    • Provide assistance to clients in completing intake forms, Medicaid Applications, Medication Assistance Forms, Social Security Applications, etc.
    • Facilitate referrals to other agencies and support organizations and ensure that they are appropriate and arranged in a timely manner. Will coordinate care with out-patinet services.
    • Maintain community (local, regional, and state) resource contact information for referral purposes.
    • Documents interventions and interactions with patients and families in the medical record.
    • Assists in training for staff.
    • Conducts In-Patient support/educational groups for patients.
    • Attend staff meetings, training sessions, and other meetings and activities as directed.
    • Must be free from governmental sanctions involving health care and/or financial practices.
    • Complies with the hospital’s Corporate Compliance Program including, but not limited to, the Code of Conduct, laws and regulations, and hospital policies and procedures.
    • Performs other duties as assigned.
  • JOB SPECIFICATIONS
    • 1. Education: Bachelors in Psychology, Human Services or related field, or commensurate experience considered.
    • 2. Licensure: Not required.
    • 3. Certifications required: See Cardiopulmonary Resuscitation Certification Policy and Certifications/Education Requirements Policy.
    • 4. Experience: 2 years hospital experience or case management experience required without Bachelors.
    • 5. Essential Technical/Motor Skills: Able to enter material into Anasazi computer system.
    • 6. Interpersonal Skills: Able to meet and relate to clients, BHS staff, CCMH staff, employees of community agencies and members of the public at large, in a manner that facilitates delivery of clinical services and presents a positive perception of BHS and CCMH.
    • 7. Essential Physical Requirements: Must be able to perceive and comprehend spoken and written English language. Must be able to manipulate computer, fax, phone, copy machine. Must be able to speak in clear, understandable and appropriate manner.
    • 8. Essential Mental Abilities: Must be able to assess, appraise, evaluate, and understand complex and technical mental health concepts and client functioning. Must be able to communicate to clients, professionals, employees and the public at level appropriate to the circumstances.
    • 9. Essential Sensory Requirements: Hearing and vision must be correctable to a level which makes perception and understanding of written and spoken language easily accomplishable.
    • 10. Exposure to Hazards: OSHA CATEGORY III. TASKS THAT INVOLVE NO EXPOSURE TO BLOOD, BODY FLUIDS, OR TISSUES AND CATEGORY I TASKS ARE NOT A CONDITION OF EMPLOYMENT. The normal routine involves no exposure to blood, body fluids, or tissues (although situations can be imagined or hypothesized under this where anyone, anywhere, might encounter potential exposure to bodily fluids). Persons who perform these duties are not generally called upon as part of their employment to perform or assist in emergency medical care or first aid.
