Assists in meeting the psychosocial needs of patients and enabling patients to utilize medical care in order to achieve their optimal level of health. Assists with the discharge planning process via in-house consults from members of the healthcare team. Assistance may include evaluation, referrals to community resources, interface with agencies such as DHR. Assists with needs of impoverished patients including but not limited to assistance with medications and placement. Assists in adoption of infants on the Women’s and Children’s unit per organizational policy upon request. Works closely with victims of abuse and/or neglect, makes appropriate referrals to the Department of Human Resources, and provides appropriate resources. Assists with DHR guardianship when needed. In addition, DHR referrals, suspected or actual abuse cases, and impoverished patients with difficult dispositions may require the assistance of the social worker in the acute care setting. Assists with the processing of any patients in all care settings: serves as a resource for social needs through out the organization. Interacts with other social workers within the medical center. Master’s level: serve as a clinical resource to the social work program through out the facility; audit social work processes and patient records to assure standards of social work care are met; provide documentation of audits, action plans and follow up regarding any deficit found during audit, receive consults from social workers in need of assistance with complex cases: other services as identified.
Education REQUIRED: Graduation from an accredited school of Social Work with a minimum of a Master’s degree and current licensure in the State of Alabama required.
Experience REQUIRED: Excellent communication skills (oral and written) required. Continuing education courses/competency validation in areas which update knowledge and expertise in discharge planning, financial assistance and community resources required. Working knowledge of Social Work theory and practice required.
Licenses/Certificates REQUIRED: Licensed Graduate Social Worker (LGSW) or Licensed Certified Social Worker (LCSW) required.
