Social Worker MSW

Position Requirements :

  • Master’s degree in Social Work.
  • Current unrestricted licensure in the State of New Mexico to practice as a MSW.
  • A minimum of three years working in a home health and/or hospice care environment preferred or five years of clinical experience.
  • Proven leadership skills and the ability to build a team approach to patient care.
  • Ability to deal with problems involving several concrete variables in standardized and non standardized situations.
  • Integral part of the IDG in establishing the plan of care.
  • Social work services are provided by a qualified social worker who has clinical experience relevant to counseling and casework needs of patients and families, in accordance with the plan of care.

ESSENTIAL FUNCTIONS:

  • Adheres to agency, state, and federal regulations.
  • Demonstrates knowledge and skill of the agency complaint, abuse, neglect and exploitation process and state/federal guidelines and DOH Incident Reporting Policy and Procedure.
  • Assists in the training of the hospice support staff in care delivery to the hospice patient and family.
  • Identifying the patient and family’s psychosocial needs and coping skills.
  • Providing interventions for specific psychosocial symptom relief as needed: enhancing the strengths of the family system.
  • Consulting, collaborating, and contributing to the development of a plan of care by the IDG for all patients on hospice service.
  • Assessing bereavement risk, and referring for ongoing bereavement services
  • Providing holistic family-centered care across treatment settings and bereavement follow-up for high-risk family members/caregivers; screening for psychopathology and educating and intervening as necessary.
  • Connecting the patient, family, and caregiver with community resources
  • Documenting psychosocial assessment, goals, care provided, and patient/family response to interventions.
  • Involving the patient and family in the treatment plan.
  • Participating in in-service programs.
  • Strong organization and time management skills.
  • Proven understanding of clinical assessment including use of Quality Assurance Program (QAPI), community resources, bereavement and pastoral counseling.

Salary is DOE.
