GENERAL SUMMARY
The Youth Services Case Manager will be responsible for overseeing and/or providing services to targeted children/adolescents and their
families.
ESSENTIAL JOB RESPONSIBILITIES
Assessment of an individual’s needs, strengths and weaknesses.
Schedule and Develop the Individualized Treatment Plan.
Linkage or referral of individuals to resources and services both within the agency and among the interagency
network.
Monitoring the implementation of an individual’s Treatment Plan to assure services are being provided, as
planned.
Advocacy on behalf of the individual to assure accessibility to services.
Meet with the client, his/her family members, caregivers and relevant others face to face to ascertain his/her
strengths and needs through the home visit process.
Schedule and convene annual Treatment Plan and Treatment Plan updates.
Assure the availability of necessary educational services, request school grades and make regular contact face to
face at the student’s school.
Participate in on-call crisis rotation.
Document all case management activities conducted on behalf of the client.
Provide behavioral support plan, development, review and modifications as directed by the Treatment Team
Leader.
Prepare and implement family visitation plans between foster children/adults and natural family members if
applicable.
Attend judicial review, custody reviews and court hearings when necessary.
Work closely with the referring entity to ensure proper case coordination.
Complete monthly summaries to referring entity.
Provide transportation to clients, natural family members when necessary.
Recruit foster parents and kinship caregivers and evaluate their suitability as caregivers.
Help foster parents and kinship caregivers effectively parent children who are not their own.
Work cooperatively using a team approach.
Involve the biological parents in as many decisions about their child’s day-to-day care as possible.
Help biological parents improve their parenting skills.
Provide services that respect each child and family’s cultural background, heritage and identity.
Help children adjust to transitions involved in separation and placement; and evaluate the impact of services
provided for the child and biological family.

BSW or BA in Human Service field.
One year (1) in child welfare experience
Must have a valid driver’s license.
LSW, Provisional or Temporary Social Work License. CPR, CPI and First Aid

OSHA EXPOSURE DETERMINATION: OSHA EXPOSURE DETERMINATION: The Case Manager’s OSHA determination is category II.
Their tasks normally involve no exposure to blood, body fluids or tissues, but there is a potential for exposure and appropriate protective
equipment would be available. Personnel with this classification are not required to wear protective equipment but must have it available
and must be willing to use it.
LANGUAGE SKILLS: Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure
manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of
organization.
MATHEMATICAL SKILLS: Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, percentages,
area, circumference, and volume. Ability to apply concepts of basic algebra and geometry
REASONING ABILITY: Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited
standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram
