The Social Worker completes psychosocial assessments and participates in treatment team meetings; provides individual and/or family conferences; screens patients on admission for transition of care; acts as a liaison with community resources/agencies: reports abuse and neglect to DHR; maintains a current list of community agencies/resources; assigns primary care physicians as needed; links patient with community resources i.e. nursing home placement, home healthcare, hospice, transportation, as well as plans, organizes, coordinates, and implements comprehensive social work services to assigned patients.
Required Skills
Required Experience
Bachelor's degree in Social Work from an accredited college or university and a current Alabama Social Workers license required. Master's degree in Social Work preferred. Minimum of six months acute care experience preferred. Psych experience preferred. Basic Life Support Certification by the American Heart Association.
