The RN Case Manager serves as a longitudinal advocate for members across the continuum of care, including hospital admissions, transitions from acute care facilities and to the next level of care such as the member’s home, as well as facilitation of communication with the primary care physician (PCP). The RN Case Manager assists in the coordination of care, develops a comprehensive care plan and provides assistance and resources for the member within the community setting to help members remain in their home. Working closely with the patient, the RN Case Manager provides support and education for the patient to manage their own health.
Primary Responsibilities:
- Collaborates effectively with the interdisciplinary team (IDT) to complete an individualized comprehensive plan of care including facilitating transitions of care.
- Provides assessments of physical, psycho - social and transition needs within the patient’s actual settings not limited to the PCP office, hospital, SNF and / or patient’s home.
- Develops interventions and processes to assist the patient and care givers in meeting short and long term goals.
- Initiate appropriate community - based referrals.
- Demonstrates and implements case management principles, processes and current standards of care when creating and implementing individualized plan of care and interventions.
Required Qualifications:
- Registered Nurse with a current, unrestricted license to practice in the state of Utah
- 2+ years of case management experience
- 3+ years of fast paced background in acute care, home health, hospice, geriatric and /or community settings
- Proficient knowledge of chronic diseases; COPD, Asthma, Diabetes, CHF, Depression, PVD and CAD
- Knowledge of discharge planning options and interdisciplinary approaches to decreasing post - discharge complications and avoidable readmissions to the acute care setting for OptumCare - Utah members
- One of the following certifications: ACM, CCM, CMC, and / or CMCN
- If not certified in Case Management, required within first year of employment
- Bachelor’s degree
- Experience working with Medicare population and individuals with multiple co - morbidities
- Experience working with Centers for Medicare and Medicaid Services, especially Medicare and Medicare Advantage programs
