Case Manager - Navigator Job Opening

Responsibilities: schedule clients for intake, safety planning; create profile in our database program; assess need for services; create service plan. Conduct linkage referrals with on-site partner agencies as well as community service providers; document all service delivery. Attend trainings as needed to ensure competency in serving DV victims and their families. Hold in confidence all client-related information.

Qualifications: Knowledge of domestic violence and trauma-informed practices, philosophy and interventions. Ability to work with diverse groups. Minimum 2 years experience in the field of social work preferred.

Education: Bachelor's Degree preferred. Must complete fingerprinting process upon hire and complete 40-Hrs DV training with in the first 3 months of employment.
